Comparación entre las escalas Heart y Grace en relación con hallazgos en la coronariografía de pacientes con síndrome coronario agudo sin elevación del segmento ST

dc.description.abstractObjetivo: Evaluar comparativamente el rendimiento diagnóstico de las escalas HEART y GRACE en relación con hallazgos en la coronariografía de pacientes con SCASEST en un Hospital Universitario de Barranquilla, Colombia. Métodos: Se realizó estudio retrospectivo, longitudinal, observacional, con interés analítico. Resultados: La muestra estuvo representada por 131 pacientes, en donde el 61,8% tuvieron una edad > 60 años, predominando el sexo masculino con 52,7%, siendo el principal factor de riesgo la hipertensión arterial esencial con 87%. Según hallazgos de la coronariografía el 55,7% presentó estenosis significativa. De acuerdo con la escala HEART, el 48,9% de los pacientes se clasificó en categoría de riesgo alto y en la escala GRACE el 64,9 % se ubicó en riesgo bajo. En cuanto a eficiencia de ambas escalas, la escala HEART mostró una sensibilidad del 59,3% frente a un 5,8% de la escala GRACE, y en lo que respecta a especificidad, la escala GRACE obtuvo un valor del 97,8% frente a la escala HEART, el cual correspondió al 71,1%. El ABC en la escala GRACE fue de 0,53 (IC 95% = 0,42 – 0,63) no llegando a discriminar correctamente a los pacientes con enfermedad coronaria, mientras que la escala HEART presentó un mejor desempeño con una ABC de 0,68 (IC 95% = 0,58 – 0,79). Conclusiones: Este estudio demostró la superioridad del puntaje HEART en cuanto a predicción de enfermedad coronaria significativa con respecto al puntaje GRACE.spa
dc.description.abstractenglishTo comparatively assess the diagnostic yield of the HEART and GRACE scales concerning findings in the coronary angiography of patients with NSTEMI at a University hospital in Barranquilla, Colombia. Methods: A retrospective, longitudinal, observational, analytical study was conducted. Results: Of 131 patients, 61.8% were over 60 years of age, predominantly male with 52,7%; the main risk factor was essential arterial hypertension with 87%. According to coronary angiography findings, 55,7% had significant stenosis. According to the HEART scale, 48,9% of the patients were classified in the high-risk category. On the other hand, with GRACE scale, 64.9% were at low risk. Regarding the efficiency of both scales, in the HEART scale a sensitivity of 59.3% was obtained compared to 5.8% in the GRACE scale, and about specificity, the GRACE scale obtained a value of 97, 8% compared to the HEART scale by 71.1%. The AUC in the GRACE scale was 0.53 (95% CI = 0.42 - 0.63), without correctly discriminating patients with coronary heart disease, while the HEART scale presented a better performance with a AUC of 0.68 (95% CI = 0.58 - 0.79). Conclusions: The present study demonstrated the superiority of the HEART score in predicting significant coronary disease over the GRACE score.spa
